Most of the items listed here have been sent home or are available in Registration Packets. SHS realizes that you are asked to provide the same information in a variety of forms. We have streamlined all forms and all information we need from parents. We must, however, provide different forms in different offices and files. We ask your patience and forbearance in completing all that are needed. Please observe the deadlines, if any, noted on the forms.
