
Several forms are required to ensure that your child is healthy and that all
immunizations are up-to-date. State of New Hampshire law requires compliance
with certain health-related regulations to protect both your child and the other
children in our school.
Emergency Information Record
This form should be given priority by
parents and guardians. The information you and your child’s physician provide
will help SHS better respond to your child’s needs should an emergency situation
arise. Please note that if the child is to carry medication for
self-administration, then a physician's signature is required.

Child Health Form
The
top portion of this form is to be completed and signed by the parent or
guardian. The remainder of the form is to be completed by the physician. Many
physicians have the capacity to furnish a computer printout of your child’s
health record. If your physician does this, simply complete Part 1 and attach
the physician’s printout. Use this Child Health Form as a guide to ensure that
all information is provided by the physician.

Individual Health Care Plan –
Children with Allergies
SHS provides both prevention and action plans to
care for children with allergies. If your child has an allergy, please review
this form with your physician and request the physician’s signature. The more we
know about your child’s allergic reactions, the better we can assist in an
emergency. In an emergency, SHS will follow the
Food Allergy Action Plan.

Immunization Requirements
The following are the State of New
Hampshire immunization requirements for children entering the indicated grade
level. Pease review the documents that apply to your child(ren) and
consult with your physician about the proper immunization for your child’s age
and grade:
Potassium Iodide Information and Consent Form
Because of the Town of
Hampton’s proximity to the Seabrook Nuclear Power Plant, SHS asks parents to
review the reason for administering a potassium iodide tablet and then to
indicate whether SHS has your permission to do so.
