
AAll Sacred Heart School enrollment forms are accessible
here online. Depending on your preference, you may either print out the
enrollment forms, complete them, and mail them to SHS, or you may come
to our office and pick up an enrollment packet or complete the
enrollment in person. Regardless of your choice, we will be pleased to
accommodate your needs as you prepare to enroll your child.
Student Registration Form
The first step in registering your child(ren) for
Sacred heart School is to complete a Registration Form and submit it
with the Registration Fee. This Form may be used to register for either
the current or the new school year. Once the Registration is complete,
Sacred Heart School will notify you about the next steps in the process.
If you are using this online form and have not yet visited SHS, please
arrange for a tour so that you and your child(ren) will be able to see
our school in action.

Request for Copy of School Records
If your child is
transferring to SHS from another school, you must request that a copy of
all school records be sent to SHS. Present this form to the
administrators of your child’s school. Your signature indicates that the
school has your permission to release your child(ren)’s school records
to SHS. After acceptance, a Request for All School Records should be sent so the
original documents are on file at SHS.

Student Assessment Information
Students entering SHS are
screened to ensure appropriate placement of the student and to assess
any individual needs the student might have. Complete and submit this
form to assist SHS in the total assessment process.

Financial Aid Supplement Form
If your family is planning to request Financial Aid,
your request will be made through the FACTS Tuition Management Company
(www.factsmgt.com).
The FACTS Grant and
Aid Assessment and the SHS Financial Aid Supplement forms are completed
as part of this process and
will be provided to all parents at the time of
enrollment or registration. All financial and financial aid
information is held in strict confidence. For budgeting purposes, SHS asks that you complete the forms and submit
them by April 18 to help with the planning
process.
Enrollment Contract OR Supported Student Enrollment Contract
An Enrollment Contract indicating your tuition payment option is requested
by May 14. All children from your family who are or will be attending SHS in 2007-2008 should be included on this form. If your family is
eligible for reduced tuition based on your participation in the life of
your parish, you are asked to complete the Supported Student Enrollment
Contract.
