New Students and all students in PreK3 and PreK4:
All required medical documentation must be completed and submitted to the school before the start of the year for new students and students in PreK3 and PreK4.
Returning Students:
Paperwork for returning students must be submitted in the first week of school.
Health Information:
All required medical documentation (excluding physicals) should be updated with every physical or in the event any changes have occurred to the student’s diagnosis. All students are required to submit a physical form signed and dated by a doctor. This will be valid for two years; however, we encourage physicals to be turned in yearly. No physical may hit the 2-year mark during the school year. Therefore, all physicals for the 2024-25 school year must be dated 6/30/24 or newer. Physicals should be provided before the beginning of school.
These forms need to be updated yearly:
Student Information Form
Student Health Information Form (also include a copy of latest physical)
Over the Counter Medication Permission Form (if applicable)
Physician Medication Permission Form (if applicable)
Acceptable Use Policy
Extended Care Form
